Publisher's Synopsis
This Journal is designed for individuals who want a better understanding of their medical situations. I personally tested this journal for my own chronic illnesses, and used it with my doctors in multiple hospitals and several states to coordinate care. To get the journal to where you see it today took a 5-year period of testing and evaluating. Before I released it to the public, I asked several trusted individuals with a range of life experience to test it. I organized a small panel of individuals who could help me understand how this book would make life better for those struggling. My panel included a PhD level NASA research scientist who herself suffers from medical conditions, a second year student in a Registered Nurse Program dealing with chronically ill patients, a mother who was suffering with chronic depression and raising a son with disabilities, an executive who helps write health care coding for the State of Maine, and finally a therapist who has treated patients for over 10 years. The culmination of their recommendations and my work is what you see before you right now. The Journal has shown promise in helping with: -Chronic Illnesses -Depression -Diabetes -PTSD -Dealing with Multiple Medications -Managing Allergies to Medication -Watching for Medication Interactions -Managing Allergies to the Environment -Coordinating Care Across Multiple Hospitals -Coordinating Care Between Doctors In all things the patient is their own best advocate and this journal will help them to do that."
