This text provides a guide to identifying nursing diagnoses and patient care plans. It is organized into three sections. Section one explains how the nurse arrives at a nursing diagnosis using assessment findings as well as other known data; section two contains an alphabetical list of approximately 1000 client signs and symptoms, medical diagnoses, therapeutic modalities, nursing diagnoses and psychiatric diagnoses; and section three provides definitions, defining characteristics, client/goal outcomes; suggested nursing interventions with rationales, and client/family teaching protocols for all NANDA-approved diagnoses.
