Section One, "Nursing Diagnosis and the Nursing Process", explains how the nurse arrives at a nursing diagnosis using assessment findings and other known data such as medical diagnosis. The authors then explain the five steps taken to make a nursing diagnostic statement and provide guidelines for determining patient outcomes, interventions and evaluation criteria for each diagnosis.;Section Two, "Guide to Nursing Diagnoses", contains an alphabetical list of approximately 1000 client signs and symptoms, medical diagnoses, therapeutic modalities, nursing diagnosis and psychiatric diagnosis. Accompanying each entry is a list of potential nursing diagnoses with appropriate "related to" statements. Section Three, "Guide to Planning Care", provides definition, defining characteristics, client goals/outcomes and suggested nursing interventions for all NANDA-approved diagnoses.
