This text provides a guide to identifying nursing diagnoses and patient care plans. It is organized into three sections: "Nursing Diagnosis and the Nursing Process" explains how the nurse arrives at a nursing diagnosis using assessment findings as well as other known data; "Guide to Nursing Diagnoses" contains an alphabetical list of approximately 1000 client signs and symptoms, medical diagnoses, therapeutic modalities, nursing diagnoses and psychiatric diagnoses; and "Guide to Planning Care" provides definitions, defining characteristics, client goals/ outcomes and suggested nursing interventions.
